29-Year-Old Male with Androgenetic Alopecia Undergoes PRP Hair Restoration in Katy, Texas

Background

A 29-year-old male presented to our dermatology clinic with concerns of progressive hair thinning over the past five years. His hair loss was diffuse and gradual, primarily affecting the frontal and vertex scalp. The patient reported a strong family history of male pattern hair loss, with both his father and brother experiencing similar symptoms. He currently washes his hair three times weekly and has not pursued any previous treatment for hair loss.

Examination and Diagnosis

A thorough scalp and hair examination was performed using dermatoscopy. The findings were consistent with androgenetic alopecia (AGA) — commonly referred to as male pattern hair loss. The hair pull test was positive, confirming ongoing shedding.

To confirm the diagnosis, a punch biopsy was discussed as an optional diagnostic step. The patient was counseled that androgenetic alopecia is genetically driven and progressive, and that early treatment offers the best opportunity to preserve existing hair and stimulate regrowth.

Treatment Discussion

During consultation, Dr. Reena Jogi reviewed multiple evidence-based treatment options available at Village Dermatology in Katy and Houston, Texas, including:

  • Oral minoxidil (low-dose therapy for systemic hair stimulation)

  • Finasteride (to address hormonal causes of male pattern hair loss)

  • Topical minoxidil (Rogaine®)

  • Low-level laser therapy (LLLT)

  • Hair growth supplements such as Nutrafol®, Viviscal®, and Votesse®

  • Platelet-Rich Plasma (PRP) injections

  • AlmaTED® noninvasive hair restoration treatment

  • Hair transplantation for advanced cases

After a detailed discussion of risks, benefits, and expectations, the patient elected to proceed with PRP therapy for hair restoration.

Platelet-Rich Plasma (PRP) Procedure

PRP therapy uses the patient’s own blood to isolate platelet-rich plasma, a component rich in growth factors that stimulate hair follicle activity and promote regrowth.

The procedure involved:

  1. Drawing the patient’s blood via sterile venipuncture.

  2. Processing the sample in a centrifuge to separate the PRP layer.

  3. Injecting approximately 7 cc of PRP into the scalp’s subdermal layer.

To ensure comfort, Pro-Nox® (nitrous oxide/oxygen mixture) was used for mild sedation and pain relief. The patient tolerated the procedure well, with no complications noted.

Aftercare and Follow-Up

Post-procedure instructions included:

  • Washing hair gently with sulfate-free shampoo after 24 hours.

  • Avoiding hair coloring for 2–3 days.

  • Continuing prescribed or topical hair loss treatments such as minoxidil or supplements.

The patient was advised to return in one month for the second PRP session, as optimal results typically require a series of 3–4 treatments, spaced one month apart.

Discussion

This case highlights a classic presentation of male pattern hair loss in a young adult with a strong genetic predisposition. PRP offers a non-surgical, regenerative approach that can enhance follicular function and improve hair density. When combined with oral or topical medications, PRP can significantly enhance overall treatment outcomes.
