Addressing Long-Term Hair Loss in Women: A Case Study on Androgenetic Alopecia with PRP and Dutasteride

Patient Case Study:

A 56-year-old female presented to our dermatology clinic in Houston, Texas, with a chief complaint of gradual and diffuse hair loss on her scalp. The condition had been progressing over the past seven years. Despite consistent use of Rogaine 2% and Nutrafol supplements, she reported minimal improvement. A family history of maternal hair thinning was also noted.

Clinical Evaluation:

Upon examination, the patient appeared well-developed and well-nourished. A thorough dermatoscopic scalp evaluation confirmed signs of androgenetic alopecia (AGA)—a common yet often emotionally distressing cause of female pattern hair loss. A positive hair pull test further supported the diagnosis.

Given the chronicity and resistance to her current regimen, a comprehensive discussion of advanced treatment strategies was conducted.

Tailored Treatment Plan:

We carefully reviewed a range of evidence-based therapies for AGA, including:

After an in-depth discussion of the risks, benefits, and expected outcomes, the patient elected to switch from Rogaine 2% to Rogaine 5%, initiate oral Dutasteride 0.5 mg daily, and begin a series of PRP treatments for non-surgical hair restoration.

PRP Treatment:

The patient underwent her first PRP treatment the same day. After sterile venipuncture and centrifugation of her blood, 7cc of PRP was injected into the subdermal scalp. Pain management was achieved using Zimmer cooling. The procedure was well-tolerated with no complications.

She was advised on post-procedure care, including:

  • Gentle shampooing with sulfate-free products

  • Avoiding hair dye for 2–3 days

  • Continuing any prescribed hair growth therapies

Looking Ahead:

The patient is scheduled to return in one month for her second PRP session. Continued use of oral dutasteride and topical minoxidil is recommended to support long-term follicular health. We will monitor her response to treatment closely and adjust as needed.
