“Can You Still Get Acne While on Birth Control?”

A 24-year-old female patient recently came to our clinic concerned about ongoing acne breakouts. She explained that her acne had been present for several years, primarily affecting her face, and she was already taking birth control pills (BCP)to help manage it.

To answer this, we performed a detailed skin evaluation and discussed treatment options.

Understanding Adult Acne

During the dermatology exam, we identified:

Acne develops when several factors occur simultaneously:

  • Excess oil (sebum) production

  • Clogged hair follicles

  • Bacterial growth (Cutibacterium acnes)

  • Inflammation in the skin

Hormonal fluctuations can worsen these factors, which is why many women experience acne even while using hormonal birth control.

Why Birth Control Alone May Not Fully Treat Acne

Birth control pills can help regulate hormones that contribute to acne, but they often do not address every cause of acne.

Additional contributing factors may include:

  • Genetics

  • Excess oil production

  • Skin bacteria

  • Pore blockage from dead skin cells

  • Cosmetic products that clog pores

Because acne has multiple causes, treatment usually requires combination therapy.

Treatment Plan for Moderate Acne

After discussing treatment options with the patient, we recommended a topical treatment regimen.

The patient elected to start with:

Topical Tretinoin (Retinoid)

Tretinoin works by:

  • Increasing skin cell turnover

  • Preventing clogged pores

  • Improving existing acne lesions

  • Helping reduce acne scarring over time

Patients are instructed to:

  • Apply a pea-sized amount at bedtime

  • Start 2–3 nights per week

  • Gradually increase to nightly use as tolerated

Because retinoids can cause dryness, it is important to apply them after the skin is completely dry.

Topical Clindamycin Lotion

Clindamycin helps treat acne by:

  • Reducing acne-causing bacteria

  • Decreasing inflammation

The patient was instructed to apply clindamycin lotion in the morning.

Recommended Skincare for Acne-Prone Skin

Proper skincare can significantly improve acne treatment outcomes.

We recommended using non-comedogenic skincare products, which means they will not clog pores.

Cleansers

Examples include:

  • CeraVe Foaming Cleanser

  • Cetaphil Oil Control Cleanser

  • Neutrogena Oil-Free Acne Wash

  • SkinMedica AHA/BHA Cleanser

Moisturizers

Even acne-prone skin needs hydration.

Recommended options include:

  • CeraVe PM Moisturizer

  • Cetaphil Oil Control Moisturizer

  • EltaMD UV Clear

Sunscreen

Daily sunscreen is essential, especially when using retinoids.

Patients should use broad-spectrum sunscreen SPF 30 or higher to prevent irritation and protect the skin.

How Long Does Acne Treatment Take to Work?

Many patients expect rapid results, but acne treatments require patience.

Patients were counseled that:

  • It can take 2–3 months to see significant improvement

  • Most patients experience 60–80% improvement with consistent treatment

  • Early treatment can help prevent permanent acne scars

If acne does not improve after several months, additional treatments may be considered.

Additional Acne Treatment Options

If needed, dermatologists may recommend other therapies such as:

  • Spironolactone for hormonal acne

  • Oral antibiotics for inflammatory acne

  • Isotretinoin (Accutane) for severe acne

  • Prescription topical combinations
