Case Report: Platelet-Rich Plasma (PRP) Therapy for Androgenetic Alopecia

At our dermatology practice in Houston and Katy, Texas, we treat many patients with androgenetic alopecia (pattern hair loss) who seek advanced, non-surgical options for hair restoration. This case highlights the role of platelet-rich plasma (PRP) injections in managing long-standing male hair loss.

Patient Presentation

The patient presented with a history of diffuse, non-scarring hair loss consistent with androgenetic alopecia (AGA). His thinning was most pronounced at the vertex and frontotemporal regions—a hallmark pattern of male hair loss.

Clinical Examination

  • Diffuse thinning without signs of scarring

  • Patterned hair loss involving the crown and temples

  • Otherwise normal scalp and skin examination

  • Patient appeared well-nourished, alert, and in no distress

Counseling and Treatment Options

The patient was counseled extensively on the nature of androgenetic alopecia:

  • A genetically pre-determined, slowly progressive condition

  • Male hair loss typically affects the crown and frontotemporal scalp

  • Early intervention can help preserve density and slow progression

Treatment options discussed included:

  • Topical minoxidil 5% solution

  • Oral finasteride

  • Supplements (Viviscal, Nutrafol, Votesse)

  • Low-level laser therapy

  • PRP injections

  • Hair transplantation

After reviewing risks, benefits, and expectations, the patient elected to continue topical minoxidil and proceed with PRP injections.

Platelet-Rich Plasma (PRP) Procedure

  • This was the patient’s ninth PRP session.

  • Blood was drawn via sterile venipuncture and centrifuged to separate plasma rich in platelets.

  • 7 cc of PRP was injected into the scalp in a subdermal fashion.

  • Pain control was achieved with a Zimmer cooling device.

  • The procedure was well tolerated with no complications.

Post-procedure instructions included:

  • Wash hair in a warm shower the same day

  • Use sulfate-free shampoo

  • Continue topical minoxidil and supplements

  • Avoid coloring hair for 2–3 days

Conclusion

This case demonstrates how PRP therapy can be an effective adjunct in treating male pattern hair loss, particularly when combined with other therapies such as topical minoxidil. For many patients, a multimodal approach offers the best chance of maintaining and improving hair density.
