Comprehensive Dermatology Care for Men: Hair Loss, Mole Evaluation, and Acne Scar Consultation in Katy & Houston, Texas

This case highlights a 31-year-old male who presented for evaluation of androgenetic alopecia, a mole on the forearm, and acne scarring.

Patient Presentation

A 31-year-old male presented as a new patient with three primary concerns:

  1. Progressive hair loss on the scalp

  2. A mole on the right forearm

  3. Cosmetic consultation for acne scars

The patient reported years of gradual, generalized scalp hair thinning, moderate in severity, without itching, pain, or scalp inflammation. He had been using topical minoxidil (Rogaine) consistently for several years with partial improvement. A positive family history of male pattern hair loss was noted in his father.

Additionally, the patient requested evaluation of an asymptomatic mole on the right forearm, present for several months, and expressed interest in cosmetic treatment options for acne scarring.

Clinical Examination

A comprehensive dermatologic examination was performed, including:

  • Scalp and hair evaluation

  • Examination of the face and eyelids

  • Inspection of both forearms

The patient was well developed, well nourished, alert, oriented, and in no acute distress.

Mole Evaluation: Benign Nevus

On examination, the lesion on the right forearm appeared as a regular, symmetrical, evenly pigmented nevus, consistent with a benign mole. Similar benign nevi were also noted on the left forearm.

The patient was reassured regarding the benign nature of the lesion. He was counseled on monthly self-skin exams and advised to return for evaluation if any mole changes in size, shape, color, or symptoms such as itching or bleeding. Given his family history of non-melanoma skin cancer, annual professional skin exams were recommended.

Diagnosis: Androgenetic Alopecia (Male Pattern Hair Loss)

Scalp examination revealed diffuse, non-scarring hair thinning with a patterned distribution, consistent with androgenetic alopecia (AGA). The diagnosis and natural progression of male pattern hair loss were reviewed in detail.

The patient was counseled that AGA is genetically determined and slowly progressive, commonly affecting the vertex and frontotemporal scalp in men.

Hair Loss Treatment Plan

After discussing all available options, the patient elected to initiate oral minoxidil while continuing topical therapy. Baseline clinical photographs were obtained for future comparison.

Treatment options reviewed included:

  • Oral minoxidil

  • Topical minoxidil 5%

  • Finasteride

  • Low-level laser therapy (LLLT)

  • Hair growth supplements (Viviscal, Nutrafol, Votresse)

  • Platelet-rich plasma (PRP) injections

  • AlmaTED regenerative hair treatment

The risks, benefits, expected outcomes, and potential side effects of each therapy were reviewed extensively. Counseling was provided regarding finasteride, including discussion of rare side effects such as sexual dysfunction and mood changes, with reassurance regarding overall safety and efficacy.

The patient demonstrated understanding and comfort with the treatment plan.

Cosmetic Consultation: Acne Scars & Photoaging

During the cosmetic consultation, the patient also exhibited signs of photoaging (solar elastosis), including light tan macules in sun-exposed areas. Counseling was provided regarding sun protection and skin rejuvenation options.

The patient was started on topical tretinoin 0.025% cream to address acne scarring, uneven texture, and early photoaging. Detailed instructions for gradual introduction and proper use were reviewed to minimize irritation.

Follow-Up & Ongoing Care

The patient was advised to monitor treatment response and report any side effects. A follow-up visit in six months was scheduled to reassess hair regrowth, skin findings, and cosmetic progress.
