Female Hair Loss, Acne, and Photoaging: A Comprehensive Dermatology Visit in Katy and Houston, Texas

Many patients experience multiple skin and hair concerns at the same time, requiring a thoughtful and personalized dermatology approach. At Reena Jogi, MD, we routinely evaluate and treat hair loss, acne, and signs of skin aging in a single visit. This case highlights a 33-year-old female who presented for evaluation of hair loss, facial acne, and photoaging, with an individualized treatment plan created for each concern.

Patient Presentation

A 33-year-old female, new patient, presented with several dermatologic concerns:

  • Generalized hair loss on the scalp, moderate in severity, gradual in onset, and present for several years

  • A history of scalp bumps

  • Growths on the neck, new and untreated

  • Photoaging of the face, with long-standing sun damage

She denied a history of anemia or thyroid disease and was currently using topical minoxidil and Nutrafol® supplements.

Physical Examination

A focused examination of the scalp with hair inspection was performed. The patient appeared well developed and well nourished, was alert and oriented, and in no acute distress.

Scalp examination demonstrated diffuse, non-scarring hair thinning, consistent with female pattern hair loss. A hair pull test was negative.

Diagnosis and Management

Female Androgenetic Alopecia

The patient’s clinical presentation and history were most consistent with androgenetic alopecia (AGA). Prior laboratory testing performed at an outside dermatology office was reported as normal. She denied menstrual irregularities.

Given that the patient was not preventing pregnancy and planning family expansion, oral treatment options such as spironolactone and oral minoxidil were avoided.

Hair Loss Treatment Plan

After extensive counseling, the following options were reviewed:

The patient elected to pursue PRP therapy at a future visit. Risks, benefits, expectations, pricing, and maintenance requirements were discussed in detail. Educational handouts were provided.

Acne Vulgaris (L70.0)

The patient also presented with comedonal and inflammatory acne on the face.

Acne Treatment Plan

She was counseled on gentle, non-comedogenic skin care and prescribed:

  • Tretinoin 0.025% topical cream, applied nightly

Detailed retinoid counseling was provided, including gradual use, moisturization, and expected dryness during initiation.

Photoaging of the Face

The patient expressed concern about sun-damaged skin and early signs of aging. The benefits of topical tretinoin for collagen stimulation, texture improvement, and pigmentation were reviewed, and this therapy was incorporated into her acne and photoaging treatment plan.

Patient Counseling and Education

The patient received extensive counseling regarding:

  • The chronic and progressive nature of female pattern hair loss

  • Realistic expectations for hair regrowth and maintenance therapies

  • Consistent use of topical treatments

  • Sun protection and long-term skin care strategies

She expressed understanding and agreement with the treatment plan.

Follow-Up

  • PRP treatment to be scheduled at a later date

  • Follow-up as needed for acne, hair loss, or cosmetic concerns
