Female Pattern Hair Loss: How a 57-Year-Old Woman Began Reclaiming Thicker Hair with Oral Minoxidil and Dutasteride

Hair thinning can be frustrating, especially when it develops gradually over many years. Many women notice their ponytail becoming thinner, increased scalp visibility, or widening of their hair part long before they realize they are experiencing a medical form of hair loss.

At Reena Jogi, MD, in Katy and Houston, Texas, we recently evaluated a 57-year-old woman who sought treatment for progressive hair thinning that had slowly worsened over several years. Her case highlights one of the most common causes of hair loss in women: Androgenetic Alopecia (Female Pattern Hair Loss).

Understanding Female Pattern Hair Loss

Female Pattern Hair Loss (FPHL), also known as Androgenetic Alopecia (AGA), is the most common cause of hair thinning in women. Unlike sudden hair shedding disorders, AGA develops gradually over time due to a combination of genetic predisposition, hormonal influences, and follicular miniaturization.

Women with AGA often notice:

  • Diffuse thinning across the scalp

  • A widening part line

  • Decreased hair density

  • Increased scalp visibility

  • Preservation of the frontal hairline

Because the process is gradual, many patients delay seeking treatment until significant thinning has occurred.

The Patient's Story

This 57-year-old established patient presented with concerns about generalized scalp hair loss that had been progressing for years. The hair thinning was moderate in severity and diffuse throughout the scalp.

She had previously tried topical minoxidil (Rogaine) several years ago without achieving satisfactory results. More recently, she had started oral minoxidil approximately 2–3 weeks before her visit.

During the first week of treatment, she experienced mild swelling in her fingers, a known side effect of oral minoxidil. Fortunately, the swelling was already improving by the time of her appointment.

Recent laboratory testing was reviewed and found to be within normal limits, although vitamin D supplementation was recommended.

Clinical Examination

A comprehensive examination of the scalp, face, neck, and surrounding areas was performed.

The scalp examination demonstrated findings consistent with Androgenetic Alopecia, including diffuse thinning and reduced hair density without evidence of inflammation, scarring, or other causes of hair loss.

Based on her clinical history and examination findings, a diagnosis of Female Pattern Hair Loss (Androgenetic Alopecia) was made.

Baseline photographs were obtained to allow objective monitoring of treatment progress over time.

Treatment Options Discussed

Hair restoration is highly individualized, and multiple evidence-based treatments are available.

During her consultation, we reviewed several options, including:

Oral Minoxidil

Low-dose oral minoxidil has become an increasingly popular treatment for female pattern hair loss.

Benefits include:

  • Improved hair density

  • Increased hair shaft thickness

  • Enhanced hair growth cycle

  • Convenient once-daily dosing

Patients should understand that temporary shedding during the first 8–10 weeks is common and often indicates that the medication is beginning to shift follicles into a new growth phase.

Potential side effects include:

  • Fluid retention

  • Swelling of the hands, feet, or ankles

  • Increased heart rate

  • Dizziness

  • Unwanted facial or body hair growth

Dutasteride

Dutasteride works by reducing levels of dihydrotestosterone (DHT), one of the hormones responsible for follicular miniaturization in androgenetic alopecia.

Potential benefits include:

  • Slowing progression of hair loss

  • Preserving existing hair

  • Improving overall density

  • Supporting long-term hair maintenance

Potential side effects discussed included:

  • Breast tenderness

  • Decreased libido

  • Mood changes

  • Depression

Platelet-Rich Plasma (PRP)

PRP therapy utilizes the patient's own growth factors to stimulate dormant hair follicles and enhance hair growth.

Many patients choose PRP as an adjunctive treatment alongside medications.

AlmaTED Hair Restoration

AlmaTED is a non-invasive hair restoration treatment designed to improve scalp health and deliver growth-promoting ingredients to hair follicles.

This treatment may be beneficial for patients seeking additional non-surgical options.

Nutritional Supplements

Supplements such as:

  • Nutrafol

  • Xtrasse

  • Other physician-recommended hair supplements

may provide supportive nutrients that promote healthier hair growth.

Low-Level Laser Therapy (LLLT)

Laser caps and helmets may help stimulate follicles and improve hair density when used consistently.

The Treatment Plan

After reviewing all available treatment options, the patient elected to:

✅ Continue oral minoxidil

✅ Begin dutasteride 0.5 mg daily

✅ Consider PRP therapy in the future

✅ Consider AlmaTED treatments if additional improvement is desired

She was also advised to optimize vitamin D levels and return for follow-up evaluation in six months.

What Patients Should Know About Female Pattern Hair Loss

One of the most important concepts patients learn is that androgenetic alopecia is a chronic, progressive condition.

While there is currently no permanent cure, early treatment can:

  • Slow progression

  • Preserve existing hair

  • Improve density

  • Enhance cosmetic appearance

  • Increase confidence

The sooner treatment begins, the greater the opportunity to preserve functioning hair follicles before significant miniaturization occurs.
