Hair Restoration Success with Oral Minoxidil and PRP in a 29-Year-Old Male

This case study features a 29-year-old male with a one-year history of diffuse scalp hair loss, who achieved promising early results with oral minoxidil and platelet-rich plasma (PRP) therapy.

Patient Background

The patient, a 29-year-old male, presented with generalized hair thinning of moderate severity that began suddenly about one year prior. He reported a strong family history of androgenetic alopecia and was already using topical Rogaine (minoxidil) and Nutrafol supplements at the time of his visit. However, he was looking for more aggressive and long-term treatment options after seeing minimal improvement.

Clinical Evaluation

  • Scalp Examination: Diffuse, non-scarring patterned hair loss consistent with androgenetic alopecia.

  • General Health: Patient was well-developed, well-nourished, and in no acute distress.

  • Mood/Affect: Patient was alert, oriented, and motivated to pursue treatment options.

Diagnosis

Androgenetic Alopecia (L64.8) — Characterized by gradual hair thinning and miniaturization of hair follicles, most prominent at the vertex and frontotemporal scalp.

Treatment Plan

After a comprehensive consultation, we designed a personalized, multimodal treatment plan:

  1. Oral Minoxidil
    The patient was prescribed 2.5 mg oral minoxidil to replace topical application. Dosing began with half a tablet daily, increasing to one full tablet if tolerated. Side effects including dizziness, ankle swelling, and rapid heartbeat were discussed in detail.

  2. Nutrafol Supplements
    Continued daily use of Nutrafol to support hair growth with key vitamins, adaptogens, and DHT blockers.

  3. Platelet-Rich Plasma (PRP) Therapy
    The patient initiated a PRP series to stimulate dormant follicles and improve hair density. The first treatment involved drawing the patient's blood, centrifuging it to isolate growth factor-rich plasma, and injecting 7 cc into targeted scalp regions using a Zimmer for pain control. The procedure was well tolerated.

Post-Procedure Care

The patient was instructed to wash his hair with a sulfate-free shampoo the next day and avoid coloring treatments for 2–3 days. He was advised to continue Nutrafol and minoxidil without interruption. He is scheduled for the next PRP treatment in one month.

Discussion

Androgenetic alopecia is common in men under 30, often linked to genetic predisposition. Early intervention offers the best chance to preserve and restore hair. Oral minoxidil is increasingly recognized as a powerful alternative to topical treatments. Combined with PRP, it offers a synergistic boost to hair regrowth by extending the anagen phase and reactivating follicles.
