Managing Hair Loss in a 77-Year-Old Female: A Personalized Approach to Androgenetic Alopecia
Location: Katy & Houston, Texas
Provider: Reena Jogi MD, Hair Loss Specialist
At Village Dermatology & Hair Restoration in Katy and Houston, Texas, we recently evaluated a 77-year-old woman presenting with a chief complaint of progressive scalp hair loss. Her condition, characterized by gradual thinning over three years, was consistent with androgenetic alopecia, commonly known as female-pattern hair loss.
Patient History & Concerns
This patient, new to our practice, described her hair loss as moderate in severity and gradual in onset. She also reported occasional scalp itching and noted a family history of hair loss in her mother and aunt, indicating a strong genetic component.
Her medical history included Graves’ disease and a sensitivity to several treatment options previously tried—such as Nutrafol, Viviscal supplements, and Viviscal topical products. Due to her gastrointestinal sensitivity and history of bowel inflammation, she expressed hesitancy about oral medications. She also reported heightened stress levels related to her husband's ongoing medical issues.
Despite these challenges, the patient was highly motivated to explore safe and effective solutions to restore her hair and confidence.
Clinical Findings
During a comprehensive scalp examination, the patient showed diffuse thinning most notable in areas typical of female-pattern hair loss. Her presentation matched the clinical pattern of androgenetic alopecia, which can often be compounded by hormonal fluctuations, aging, genetics, and chronic stress.
Customized Treatment Plan
Dr. Reena Jogi emphasized a personalized, integrative treatment plan, taking into account the patient's medical history, treatment sensitivities, and preferences.
After an extensive discussion of all available therapies—including:
Topical minoxidil 5%
Finasteride and dutasteride
Spironolactone
Nutraceuticals (Nutrafol, Xtrèsse Gummies)
Low-level laser therapy
AlmaTED (non-invasive ultrasound-based scalp therapy)
The patient chose to begin AlmaTED treatments and start Xtrèsse gummies, both of which offer promising results without the use of oral medications. These choices aligned well with her lifestyle, sensitivities, and overall treatment goals.
Ongoing Support and Follow-Up
We thoroughly reviewed the treatment protocol, expected results, and costs with the patient. She plans to initiate AlmaTED treatment and was advised to follow up in six months to monitor her progress.
