Managing Postpartum Acne While Breastfeeding: A Case Report from Katy & Houston, Texas
Acne is not just a teenage condition. Many adult women experience breakouts during pregnancy and postpartum due to hormonal fluctuations. At Reena Jogi, MD, we frequently treat women in Katy and Houston, Texas who are navigating acne while breastfeeding or planning another pregnancy.
This case highlights the thoughtful management of acne in a 31-year-old female patient whose breakouts began during pregnancy and persisted postpartum.
Patient Background
The patient was initially evaluated on November 10, 2023 for acne vulgaris and prescribed tretinoin 0.025% cream, with instructions to gradually increase usage as tolerated.
She returned for follow-up reporting:
Persistent inflammatory papules and pustules
Comedonal acne (blackheads and whiteheads)
Facial distribution of breakouts
Onset during pregnancy with continuation postpartum
Importantly, she is currently breastfeeding and planning another pregnancy, which significantly influences medication safety and treatment selection.
Examination Findings
A focused dermatologic examination of the:
Face
Eyelids
Nose
Lips
Neck
confirmed ongoing inflammatory and comedonal acne. The condition was classified as inadequately controlled.
The patient was otherwise healthy, well-appearing, and in no acute distress.
Acne Treatment Considerations During Breastfeeding & Pregnancy Planning
Treating acne in women who are breastfeeding or planning pregnancy requires careful selection of medications. Many traditional acne therapies—such as oral antibiotics, spironolactone, and isotretinoin—are not appropriate during pregnancy or while trying to conceive.
We discussed safe and effective alternatives, including:
Azelaic acid
Topical clindamycin
Over-the-counter adapalene (Differin®)
Non-comedogenic skincare
The patient was advised to confirm medication safety with her OB/GYN prior to starting any new treatment, ensuring coordinated care.
Customized Treatment Plan
A pregnancy-conscious acne regimen was initiated:
Morning (AM Routine)
Gentle cleanser
Apply azelaic acid 15% gel or clindamycin 1% lotion to entire face
Apply non-comedogenic moisturizer with SPF 30+
Reapply sunscreen every 2 hours if outdoors
Evening (PM Routine)
Gentle cleanser
Apply a pea-sized amount of Differin® (adapalene) to entire face
Apply moisturizing cream
Detailed retinoid counseling was provided, including:
Use only a pea-sized amount
Wait 30 minutes after washing before applying
Use moisturizer if dryness occurs
The patient verbalized understanding of proper use and potential side effects.
Setting Expectations for Adult Female Acne
Patients should understand:
Improvement typically takes 2–3 months
Consistency is critical
Acne may fluctuate with hormonal shifts
Contact the office if acne worsens, scarring develops, or new cystic lesions appear
Adult female acne is highly treatable, but requires patience and personalized management—especially during breastfeeding or pregnancy planning.
