PRP Treatment for a 31-Year-Old Male With Long-Standing Androgenetic Alopecia in Katy & Houston, Texas

Hair loss is one of the most common concerns we evaluate at Reena Jogi MD Hair Loss Dermatology, serving patients throughout Katy, Texas and Houston, Texas. Today’s case highlights a 31-year-old male presenting with a multi-year history of progressive hair thinning who elected to begin Platelet-Rich Plasma (PRP) therapy after discussing multiple treatment options.

Patient Background

A 31-year-old male presented as a new patient with concerns of diffuse, gradually progressive hair loss over several years. He reported:

  • Moderate, diffuse thinning on the scalp

  • A strong family history of hair loss (father, maternal and paternal grandfathers)

  • Prior inconsistent use of oral minoxidil, finasteride, and dutasteride, discontinued due to worry about possible side effects

  • Current use of Nutrafol supplements consistently for 4 years

  • No scalp symptoms such as itching, burning, or pain

He denied any recent illness, hospitalizations, or thyroid-related symptoms.

Clinical Examination

A focused scalp and facial examination showed:

A dermatoscope was used to visualize follicular miniaturization—a hallmark of androgenetic alopecia.

Diagnosis

Androgenetic Alopecia (AGA)

ICD-10: L64.8 – Diffuse non-scarring patterned hair loss

The condition was explained as:

  • Genetically driven

  • Gradually progressive

  • Often worsened by stress, lifestyle changes, or inconsistent treatment

Because the patient had a long-standing history but no inflammatory symptoms, a scalp biopsy was offered but not required.

Treatment Discussion

A full review of treatment options was provided, including:

Oral Therapies

  • Oral minoxidil

  • Finasteride

  • Dutasteride

The patient declined restarting oral medications at this time due to prior concerns.

Topical Options

  • Minoxidil 5%

  • Compounded minoxidil/finasteride/tretinoin formulas

  • OTC options (Rogaine)

Regenerative & Advanced Therapies

Supplements

  • Nutrafol (already using)

  • Viviscal

  • Votesse

After discussing effectiveness, risks, costs, and expectations, the patient elected to begin PRP therapy.

Treatment Performed: PRP Injection #1

Procedure Summary

  • Written consent obtained

  • Blood drawn in sterile fashion using a butterfly needle

  • Spun in centrifuge to isolate PRP

  • 7 cc of PRP prepared in 3cc syringes

  • Injections performed throughout the scalp at subdermal depth

  • Pain minimized using Zimmer cooling device

  • No complications observed

Post-Procedure Instructions

  • Wash with warm water & sulfate-free shampoo

  • Avoid hair coloring for 2–3 days

  • Continue supplements or topical treatments if previously used

  • Expect mild scalp tenderness or pressure for 24–48 hours

Follow-Up

The patient will return in 1 month for PRP treatment #2, with typical protocols recommending 3–4 sessions spaced one month apart.

Why PRP Is an Effective Option for Men With AGA

PRP is ideal for patients who:

  • Prefer non-drug, procedural options

  • Want to avoid or reduce oral medication use

  • Are early to moderate in their hair loss

  • Have strong family history and progressive thinning

  • Wish to enhance results from supplements or topicals

PRP helps by:

  • Delivering concentrated growth factors

  • Re-activating dormant hair follicles

  • Increasing hair density and thickness

  • Reducing shedding over time
