Case Report: 26-Year-Old Female with Acne and Hair Loss

Chief Concerns:

  • Moderate cystic and nodular acne on the face

  • Gradual, generalized scalp hair thinning

  • History of scalp itching

  • Acne recurrence after prior isotretinoin course (2019)

  • Somewhat irregular menstrual cycles

  • Unwanted hair growth

She had previously tried:

She was not currently on prescription therapy.

Dermatologic Examination in Katy & Houston, TX

A full examination of the:

  • Face

  • Scalp (with hair inspection)

  • Neck and surrounding areas

Findings:

Acne:

  • Inflammatory papules

  • Nodules and cysts

  • Moderate severity

Hair:

  • Diffuse thinning

  • Pattern consistent with androgenetic alopecia (female pattern hair loss)

Given the combination of acne, irregular cycles, hair thinning, and unwanted hair growth, we discussed the possibility of underlying hormonal imbalance such as PCOS.

Diagnosis 1: Hormonal Acne (Post-Isotretinoin Recurrence)

The patient previously completed isotretinoin with good long-term control but experienced recurrence after using new skincare products.

We reviewed treatment options:

  • Repeat isotretinoin

  • Hormonal therapy (spironolactone)

  • Topical retinoids

After shared decision-making, the patient elected to start:

Spironolactone 50 mg twice daily
Topical tretinoin 0.025% nightly

Acne Treatment Plan

Morning (AM):

  1. Gentle cleanser

  2. Moisturizer with SPF 30+

Evening (PM):

  1. Gentle cleanser

  2. Pea-sized amount of tretinoin to entire face

  3. Moisturizing cream

Recommended gentle brands:

  • CeraVe

  • Cetaphil

  • Vanicream

  • La Roche-Posay

What to Expect:

  • 2–3 months for 60–80% improvement

  • Possible dryness initially

  • Gradual improvement over time

Spironolactone Counseling Provided

Discussed potential side effects:

  • Breast tenderness

  • Menstrual irregularities

  • Fatigue

  • Dizziness

  • Elevated potassium

  • Avoid during pregnancy

The patient verbalized understanding.

Diagnosis 2: Androgenetic Alopecia (Female Pattern Hair Loss)

The patient also presented with gradual worsening scalp hair thinning over several years.

Clinical findings favored androgenetic alopecia (AGA):

  • Diffuse thinning

  • Widened midline part

  • Preserved frontal hairline

  • Non-scarring

Baseline photos were taken for comparison.

Hair Loss Treatment Options Discussed

We reviewed comprehensive options available at our Katy and Houston dermatology offices, including:

Medical Treatments:

  • Topical minoxidil 5% foam

  • Oral minoxidil

  • Spironolactone

  • Finasteride (with strict pregnancy precautions)

Supplements:

  • Nutrafol

  • Viviscal

In-Office Procedures:

After discussion, the patient chose to start:

Spironolactone (addresses both acne and hair loss)

Minoxidil 5% foam was also recommended.
