Why Am I Still Breaking Out in Month 5 of Accutane? Does This Mean I Need to Stay on It Longer?
After several months of Accutane® (isotretinoin), many patients expect their skin to be completely clear. So when a new pimple appears late in treatment, it can raise an understandable concern:
“Why am I still breaking out in Month 5 of Accutane? Does this mean I need to stay on it longer?”
Recently, a 27-year-old female patient returned to our dermatology practice during her fifth month of isotretinoin treatment for facial acne. Although she had made progress, she reported some new breakouts during the previous month.
Rather than assuming her Accutane had failed, her treatment plan addressed the active lesions while continuing isotretinoin. She underwent acne extractions and elected to proceed with intralesional Kenalog (ILK) for targeted treatment while deferring a short course of oral antibiotics.
If she remains breakout-free, approximately two additional months of isotretinoin treatment are anticipated.
Her case highlights an important point for patients undergoing Accutane treatment in Houston or Katy, Texas: occasional late-treatment breakouts can affect how the final months of treatment are managed.
Still Getting Pimples During Month 5 of Accutane
This patient was taking isotretinoin 30 mg, two capsules once daily with food and water, for a total daily dose of 60 mg.
Her facial acne included:
Comedonal papules
Inflammatory papules
Pustules
At her Month 5 follow-up, she reported some new breakouts.
For patients who have already spent several months taking isotretinoin, seeing new pimples can be frustrating. However, the presence of a breakout does not by itself determine whether the overall treatment course has been unsuccessful.
Instead, the dermatologist considers the patient's overall response, frequency of new lesions, side effects, treatment duration, and cumulative dosing plan.
Is It Normal to Still Break Out in Month 5 of Accutane?
Some patients may continue experiencing occasional acne lesions later in their isotretinoin course.
Treatment response varies considerably from person to person. Some patients clear earlier, while others require additional time before they consistently remain breakout-free.
In this case, the patient had some breakouts during Month 5, so her treatment was continued rather than stopped.
The plan was particularly straightforward: if she remains breakout-free, approximately two more months of treatment may be needed.
That is why monthly follow-up remains important even when a patient is approaching the anticipated end of an Accutane course.
Does a Late Accutane Breakout Mean Treatment Isn't Working?
Not necessarily.
A single breakout or a small number of new lesions does not tell the entire story.
Dermatologists look at the overall pattern of acne activity. A patient who once developed frequent inflammatory lesions but now experiences only occasional breakouts may still have made substantial progress.
For this 27-year-old patient, the new lesions were addressed while her isotretinoin therapy continued.
Her dermatologist also discussed additional options for controlling the active acne rather than automatically changing the entire treatment plan.
Can Acne Extractions Be Done While Taking Accutane?
At this visit, the patient underwent extractions to help address some of her active acne lesions.
Acne extractions are performed to physically remove the contents of selected clogged pores or comedonal lesions.
Importantly, patients taking isotretinoin should not attempt aggressive extractions or squeeze acne at home. Isotretinoin can make the skin dry and sensitive, and manipulating lesions can increase irritation or potentially contribute to unwanted marks.
Any procedure performed during isotretinoin therapy should be discussed with and performed according to the recommendations of the treating dermatology team.
What Is an ILK Injection for Acne?
Another option discussed at this visit was intralesional Kenalog, commonly abbreviated ILK.
ILK involves injecting a corticosteroid directly into a selected inflamed lesion. It may be considered for certain inflammatory acne lesions when targeted treatment is appropriate.
For this patient, both a short course of oral antibiotics and ILK were discussed.
She preferred to defer oral antibiotics that month and proceed with ILK.
This illustrates how acne management during isotretinoin can sometimes be individualized even when the patient remains on the same systemic medication.
Why Use ILK Instead of an Oral Antibiotic?
There isn't one approach that is appropriate for every breakout.
In this patient's case, the dermatologist discussed both a short course of oral antibiotics and targeted ILK treatment. After reviewing the options, the patient preferred not to take oral antibiotics at that time and elected to proceed with ILK.
Treatment decisions depend on the number and type of lesions, severity of inflammation, medical history, current medications, and other individual considerations.
Patients should not begin leftover antibiotics or other acne medications on their own while taking isotretinoin.
How Long Will I Stay on Accutane After My Last Breakout?
This is one of the most common questions as patients approach the end of treatment.
For this patient, the plan documented that if she remains breakout-free, approximately two additional months of treatment are anticipated.
Her treatment protocol called for approximately 1 mg/kg until a cumulative dose of 200–220 mg/kg was reached.
However, isotretinoin treatment length is individualized. A dermatologist may consider the patient's clinical response, cumulative treatment, side effects, laboratory monitoring, and whether new acne lesions continue to develop.
Patients should therefore not calculate their own stopping date based solely on the number of months they have been taking Accutane.
Why Not Stop Accutane as Soon as My Skin Looks Better?
Improvement and completion of treatment are not necessarily the same thing.
By Month 5, a patient's skin may look dramatically different from when treatment began, yet occasional new lesions may still be occurring.
For this patient, continued breakouts were one reason treatment was not considered finished at this appointment.
Her dermatologist planned to monitor whether she could remain breakout-free before determining when to complete her course.
Dry Lips During Month 5 of Accutane
This patient also had cheilitis, a common problem associated with isotretinoin therapy.
Cheilitis can cause dry, cracked, irritated, or scaling lips.
She was advised to apply Vaseline or Aquaphor frequently throughout the day and before bedtime.
Patients sometimes underestimate how often lip moisturizers need to be reapplied during isotretinoin treatment. Consistent barrier protection can make dryness more manageable.
What Skin Care Should I Use During the Final Months of Accutane?
Even when acne has substantially improved, gentle skin care remains important.
This patient was counseled about:
Gentle cleansers
Non-comedogenic moisturizers
Non-comedogenic cosmetics and skin-care products
Broad-spectrum sunscreen with SPF 30 or higher
Frequent lip moisturization with Vaseline or Aquaphor
Patients should be cautious about introducing aggressive acne products simply because a few new pimples appear.
During isotretinoin treatment, the skin can be much more sensitive than it was before therapy.
Why Are Pregnancy Tests Still Needed Late in Accutane Treatment?
Pregnancy prevention remains essential throughout isotretinoin treatment because the medication can cause severe birth defects.
At this patient's Month 5 appointment, a urine pregnancy test was performed and was negative.
Her documented primary pregnancy-prevention method was abstinence, and she continued to receive isotretinoin counseling and monitoring.
Pregnancy-prevention requirements do not become less important simply because someone is approaching the end of treatment.
Patients who can become pregnant should carefully follow the current instructions provided by their prescribing dermatologist and the applicable isotretinoin risk-management program.
What Happens During the Final Months of Accutane?
As a patient approaches the end of treatment, follow-up appointments can help answer several important questions:
Are new pimples still developing?
How frequently are breakouts occurring?
Are there persistent inflammatory lesions?
Is the medication still being tolerated?
Are side effects manageable?
Has the planned treatment course been completed?
Is additional treatment time appropriate?
In this patient's case, remaining breakout-free was an important next milestone.
If her acne remained controlled, approximately two additional months of isotretinoin were anticipated.
What If I Get Another Pimple Before Finishing Accutane?
Don't assume that one new lesion means the entire course has failed—or that treatment automatically needs to be extended.
Instead, bring it up at your next appointment or contact your dermatologist if the breakout is severe or concerning.
Your dermatologist can determine whether the lesion is an isolated event or part of a larger pattern of continued acne activity.
This patient's Month 5 breakouts were managed with a combination of continued isotretinoin, extractions, and targeted ILK rather than abandoning her treatment plan.
Accutane Treatment for Adult Female Acne in Houston and Katy, Texas
Adult acne can continue well into the 20s, 30s, and beyond. For some patients with persistent or severe acne, isotretinoin may be considered when clinically appropriate.
At Village Dermatology, Reena Jogi, MD provides individualized acne evaluations and monitored isotretinoin treatment for appropriate patients in Houston and Katy, Texas.
This 27-year-old patient's case demonstrates why a few pimples during Month 5 do not automatically determine the outcome of treatment. Instead, active lesions can be evaluated individually while the overall isotretinoin course continues to be carefully monitored.
If you are still breaking out after several months of Accutane or wondering how long you should stay on isotretinoin after your acne clears, an individualized dermatology evaluation can help determine the appropriate next step.
