Why Am I Still Breaking Out on Accutane After 5 Months?

A Real Acne Treatment Success Story from Katy and Houston, Texas

One of the most common questions dermatologists hear from teenagers taking Accutane® (isotretinoin) is:

"Why am I still getting pimples if I've already been on Accutane for several months?"

If you've been wondering the same thing, you're not alone.

At Reena Jogi, MD Dermatology, serving patients throughout Katy, Houston, Fulshear, Cypress, Richmond, and surrounding Texas communities, we frequently help patients navigate the ups and downs of isotretinoin treatment. While Accutane is considered the most effective treatment available for severe acne, improvement often happens gradually rather than overnight.

Meet the Patient

A 16-year-old female returned to our office for her fifth month of isotretinoin therapy. She had initially started treatment for inflammatory and comedonal acne affecting her face and had been taking isotretinoin under close dermatologic supervision.

At her follow-up visit, she reported that she was tolerating the medication well overall. The only notable side effect was manageable lip dryness, a very common issue during isotretinoin treatment. Despite significant progress, she still experienced a few acne breakouts during the month and was concerned that her skin was not completely clear yet.

Is It Normal to Still Break Out on Accutane?

Yes.

Many patients are surprised to learn that acne can continue to improve throughout the entire course of treatment. Even after several months, occasional pimples can still occur.

Accutane works by:

  • Shrinking oil glands

  • Reducing oil production

  • Decreasing acne-causing bacteria

  • Reducing inflammation

  • Preventing clogged pores

Because these changes occur gradually, some patients continue to experience occasional flare-ups until they reach their target cumulative dose.

The good news is that most patients see substantial improvement by the end of treatment.

Managing Expectations During Treatment

One of the most important aspects of acne treatment is setting realistic expectations.

Patients are often told that it may take several months before they experience dramatic improvement. In many cases, dermatologists expect to see approximately 60% to 80% improvement after several months of consistent therapy, with continued improvement thereafter.

For this patient, the remaining breakouts were mild compared to her initial presentation, and her treatment continued with careful monitoring.

The Most Common Side Effect: Dry Lips

The patient's primary side effect was cheilitis, commonly known as dry, cracked lips. This is one of the most frequently reported side effects of isotretinoin therapy.

To help control dryness, patients are often advised to:

  • Apply Vaseline® frequently throughout the day

  • Use Aquaphor® before bedtime

  • Stay hydrated

  • Avoid licking their lips

  • Use gentle skin-care products

Most patients find this side effect manageable with consistent moisturizing.

Why Monitoring Is So Important

Isotretinoin is considered a high-risk medication because it requires ongoing monitoring throughout treatment.

Patients must:

  • Attend regular follow-up appointments

  • Complete required pregnancy prevention protocols when applicable

  • Undergo recommended laboratory monitoring

  • Report any new symptoms promptly

  • Avoid donating blood during treatment

These precautions help ensure treatment remains both safe and effective.

The Goal: Long-Term Acne Clearance

Unlike many acne treatments that only control symptoms while being used, isotretinoin aims to produce long-term remission.

Treatment is typically continued until a cumulative target dose is reached, often between 200 and 220 mg/kg depending on the patient's individual treatment plan. Reaching this goal significantly reduces the risk of future acne recurrence.

For this patient, treatment was progressing appropriately, and she continued toward her target cumulative dose under close supervision.

When Should You See a Dermatologist?

If you are struggling with:

  • Persistent acne

  • Painful cysts

  • Acne scarring

  • Acne that doesn't respond to over-the-counter products

  • Recurring breakouts despite prescription treatments

A consultation with a board-certified dermatologist can help determine whether isotretinoin or another treatment option is right for you.
