“Why is my hair shedding more even though I’ve already done PRP and take supplements?”

This case highlights how androgenetic alopecia (AGA) combined with telogen effluvium (TE) can cause sudden shedding—even after prior improvement.

Case Report: 44-Year-Old Female with Increased Hair Shedding

A 44-year-old female presented with sudden increased hair shedding over 3 months, despite ongoing treatments.

Patient History

The patient reported:

  • Sudden increase in hair shedding

  • Chronic thinning in the frontal scalp and temples

  • Use of Nutrafol supplements

  • Current spironolactone (50 mg daily) for acne

  • Prior treatments:

Despite these efforts, she noticed worsening shedding, which raised concern.

Dermatologic Examination

A detailed scalp exam and dermatoscopy were performed.

Clinical Findings:

Diagnosis: AGA with Telogen Effluvium

1. Androgenetic Alopecia (AGA)

A chronic, hormone-driven condition causing:

  • Thinning at the frontal scalp and temples

  • Progressive miniaturization of hair follicles

2. Telogen Effluvium (TE)

A temporary shedding condition triggered by:

  • Stress

  • Hormonal shifts

  • Illness or procedures

  • Medications or metabolic changes

👉 In this case, TE is likely layered on top of AGA, making hair loss appear suddenly worse.

Why Hair Shedding Can Worsen After Treatments

This is one of the most misunderstood parts of hair loss.

Even after treatments like PRP or supplements:

  • Hair cycles can shift, triggering temporary shedding (TE)

  • New treatments may cause “shedding phase” before regrowth

  • AGA continues progressing without consistent medical therapy

👉 This does NOT mean treatments failed—it often means the hair cycle is resetting.

Treatment Plan

After a detailed discussion, the patient elected to:

1. Increase Spironolactone

  • Dose increased to 50 mg twice daily

Helps:

  • Reduce androgen-related hair thinning

  • Stabilize follicle miniaturization

2. Restart PRP Therapy

PRP was initiated again to:

  • Stimulate hair follicles

  • Improve density

  • Support regrowth

PRP Details:

  • 7 cc platelet-rich plasma injected

  • Well tolerated with no complications

3. Continue/Start Minoxidil

Helps:

  • Shorten shedding phase

  • Promote regrowth

  • Improve hair density over time

What to Expect Moving Forward

Hair regrowth is gradual and requires consistency.

Timeline:

  • 4–8 weeks: shedding may continue

  • 2–3 months: early regrowth begins

  • 4–6 months: visible improvement

With combined therapy, outcomes are significantly improved.

Important Counseling Points

The patient was educated on:

Spironolactone Risks:

  • Breast tenderness

  • Menstrual irregularities

  • Elevated potassium

  • Fatigue and dizziness

👉 Requires periodic bloodwork monitoring

Telogen Effluvium Expectations:

  • Temporary condition

  • Usually resolves within months

  • Can overlap with chronic hair loss

Follow-Up Plan

The patient will return in:

1 month for PRP #2

Follow-up will focus on:

  • Shedding improvement

  • Early regrowth

  • Medication tolerance

  • Ongoing treatment adjustments
