Why Am I Suddenly Shedding So Much Hair Over the Last Few Months?

“Why am I suddenly shedding so much hair over the last few months?”

This case highlights how androgenetic alopecia (female pattern hair loss) can initially present as increased shedding and diffuse thinning.

Case Report: 35-Year-Old Female with Diffuse Hair Loss

A 35-year-old female presented as a new patient with concerns about increased hair shedding over the past four months.

Patient History

The patient reported:

  • Diffuse hair thinning across the scalp

  • Gradual onset over 4 months

  • Moderate severity

  • Hair washing approximately 3 times per week

  • Use of over-the-counter hair serums

She had also previously completed laboratory testing prior to the visit.

Dermatologic Examination in Katy & Houston, TX

A detailed scalp examination was performed using dermatoscopy.

Clinical Findings

The scalp showed non-scarring hair loss, which supports the diagnosis of female pattern hair loss.

Because of the recent increase in shedding, a punch biopsy was discussed as an option to further confirm the diagnosis if needed.

Diagnosis: Androgenetic Alopecia (Female Pattern Hair Loss)

Androgenetic alopecia is the most common cause of hair thinning in women.

It is characterized by:

  • Gradual thinning across the scalp

  • Widening of the midline part

  • Preservation of the frontal hairline

  • Miniaturization of hair follicles

Although the condition progresses slowly, early treatment helps preserve existing hair density.

Treatment Options Discussed

During the consultation, we reviewed multiple treatment options available at our Katy and Houston dermatology offices, including:

Medical Treatments

  • Oral minoxidil

  • Spironolactone

  • Topical minoxidil (Rogaine)

Supplements

  • Nutrafol

  • Viviscal

  • Votesse

Procedural Treatments

  • Platelet-Rich Plasma (PRP) therapy

  • AlmaTED hair restoration

  • Low-level laser therapy caps

We also reviewed the risks, benefits, costs, and expectations of each option.

After extensive discussion, the patient elected to begin combination oral therapy.

Treatment Plan

Oral Minoxidil

  • Dose: 1/2 tablet (2.5 mg tablet) daily

  • Helps stimulate hair growth and prolong the growth phase of hair follicles

Patients were counseled that:

  • Initial shedding for 8–10 weeks is normal

  • Results typically begin after 3–4 months

Possible side effects discussed included:

  • Dizziness

  • Swelling of the ankles

  • Shortness of breath

  • Low blood pressure

  • Increased hair growth on the face or body

Spironolactone

  • Dose: 50 mg twice daily

  • Helps reduce the hormonal influence on hair follicles

Potential side effects discussed included:

  • Breast tenderness

  • Menstrual irregularities

  • Fatigue

  • Dizziness

  • Elevated potassium levels

The patient was advised not to use this medication during pregnancy.

Additional Diagnosis: Perioral Dermatitis

During the examination, the patient also had erythematous papules around the face, consistent with perioral dermatitis.

Common triggers include:

  • Cosmetics

  • Topical steroid use

  • Fluorinated toothpaste

  • Sun or wind exposure

Perioral Dermatitis Treatment Plan

The patient was started on:

Elidel (pimecrolimus) cream – apply in the morning
MetroCream (metronidazole) – apply at bedtime

Additional recommendations included:

  • Use gentle cleansers and moisturizers

  • Avoid heavy cosmetics

  • Use non-comedogenic skincare products

  • Wear daily sunscreen (SPF 30+)

What to Expect with Hair Loss Treatment

Patients should understand that hair growth treatments require time.

Typical timeline:

  • 2–3 months: shedding stabilization

  • 4–6 months: early regrowth

  • 9–12 months: maximal improvement

Follow-up is scheduled in 3–6 months to evaluate treatment response.
