“Why Is My Hair Still Thinning Even on Finasteride… Am I Running Out of Options?”

A Real Patient Case from Reena Jogi, MD (Katy & Houston, Texas)

For many young men dealing with hair loss, starting treatment feels like taking control.

But what happens when you’re already on medication… and your hair is still thinning?

This is one of the most common—and frustrating—questions we hear at our clinic in Katy and Houston, Texas:

“Why isn’t my treatment working?”

Case Report: Persistent Hair Loss in a 20-Year-Old Male

A 20-year-old male returned for evaluation of ongoing hair thinning, despite already being on treatment.

Key History:

  • Gradual hair loss over several years

  • Currently using:

  • Unsure if there has been meaningful improvement

Clinical Findings

On examination:

Diagnosis: Treatment-Resistant Androgenetic Alopecia

This patient has male pattern hair loss, but with suboptimal response to standard therapy.

Why does this happen?

  • Not all patients respond equally to finasteride

  • Some require stronger DHT suppression

  • Others benefit from combination or systemic therapy

“Why Isn’t Finasteride Working for Me?”

This is one of the most searched questions online.

Here’s the reality:

  • Finasteride works by lowering DHT levels

  • But some patients still have enough DHT activity to cause thinning

  • Others may need more potent or additional treatments

Important: Lack of improvement doesn’t mean nothing works—it means your plan may need to be adjusted.

Updated Treatment Plan by Reena Jogi, MD

After reviewing all options, a more advanced regimen was initiated:

New Treatment Strategy:

  • Switch from Finasteride → Dutasteride 0.5 mg daily

  • Start Oral Minoxidil 2.5 mg daily

  • Continue monitoring with baseline photos

Why This Approach Works

🔹 Dutasteride:

  • Blocks more forms of DHT than finasteride

  • Often more effective in patients not responding to finasteride

🔹 Oral Minoxidil:

  • Enhances hair growth systemically

  • Can improve density when topical therapy isn’t enough

Important Counseling Points

What to Expect:

  • Initial shedding may occur (8–10 weeks)

  • Results take 3–6 months or longer

Possible Side Effects:

  • Breast tenderness

Oral Minoxidil:

Patients were advised to stop medication and contact the office if symptoms occur.
