This case highlights the progressive nature of androgenetic alopecia (female pattern hair loss) and how treatment plans often evolve over time to achieve better outcomes.

Case Report: 48-Year-Old Female with Chronic Hair Loss

A 48-year-old female presented as a new patient for evaluation of long-standing hair thinning.

Patient History

The patient reported:

Despite these therapies, she continued to experience persistent hair thinning.

Dermatologic Examination in Katy & Houston, TX

A comprehensive scalp examination was performed.

Clinical Findings

Because androgenetic alopecia can sometimes overlap with other causes of hair loss, a punch biopsywas discussed as an option for further confirmation if needed.

Diagnosis: Androgenetic Alopecia (Female Pattern Hair Loss)

Androgenetic alopecia is the most common cause of hair thinning in women.

Typical characteristics include:

  • Diffuse thinning across the scalp

  • Widening of the midline part

  • Preservation of the frontal hairline

  • Progressive follicle miniaturization over time

Even with treatment, hair loss can progress if therapy is not optimized.

Why Hair Loss Can Continue Despite Treatment

Many patients assume that if treatments like PRP or minoxidil do not produce dramatic improvement immediately, they are ineffective.

However:

  • Hair follicles respond slowly to treatment

  • Genetic hair loss is progressive

  • Combination therapy is often required

  • Treatment adjustments are sometimes necessary

For this patient, further hormonal pathway suppression was considered.

Treatment Options Reviewed

During the consultation, we reviewed a variety of hair restoration options available at our Katy and Houston dermatology offices, including:

Updated Treatment Plan

After reviewing risks, benefits, and expectations, the patient elected to proceed with:

Dutasteride

  • Dose: 0.5 mg daily

Dutasteride works by blocking DHT (dihydrotestosterone), a hormone responsible for follicle miniaturization in androgenetic alopecia.

Possible side effects discussed included:

  • Breast tenderness

  • Decreased libido

  • Mood changes

  • Depression

The patient was counseled regarding the teratogenic risk and advised to avoid pregnancy while on treatment.

Platelet-Rich Plasma (PRP) Treatment

The patient also elected to restart PRP therapy to further stimulate hair follicles.

Procedure Details

  • Blood was drawn in a sterile fashion

  • Sample centrifuged to isolate platelet-rich plasma

  • 7 cc of PRP injected into the scalp

  • Zimmer cooling device used for comfort

The procedure was completed without complications.

PRP works by delivering growth factors that stimulate dormant hair follicles and improve hair density.

Post-Procedure Care

After PRP treatment, the patient was advised to:

  • Wash hair in a warm shower

  • Use sulfate-free shampoos

  • Continue hair loss medications and supplements

  • Avoid hair dye for 2–3 days

Follow-Up Plan

Hair growth treatments require time to evaluate effectiveness.

Typical timeline:

  • 3 months – early improvement

  • 6 months – visible density changes

  • 9–12 months – maximal results

The patient will return in 1 month for PRP treatment #2.
