“Why Is My Hair Thinning Again After Stopping Spironolactone?”

This case demonstrates how female pattern hair loss can recur when medications that help stabilize hair follicles are discontinued.
Case Report: 57-Year-Old Female with Recurrent Hair Thinning
A 57-year-old female presented as a new patient for evaluation of long-standing hair thinning affecting the scalp.
Patient History
The patient reported:
Gradual hair thinning that had been occurring for several years
Moderate generalized hair loss
Prior treatment with spironolactone, which previously improved her hair density
She had stopped the medication approximately two years ago
Since discontinuing spironolactone, the patient noticed gradual worsening of hair thinning again.
Dermatologic Examination
A thorough scalp examination was performed, including dermatoscopic evaluation.
Clinical Findings
The exam revealed:
Diffuse thinning across the scalp
Positive hair pull test
Pattern consistent with androgenetic alopecia
Female pattern hair loss typically presents with diffuse thinning and widening of the midline part while maintaining the frontal hairline.
Because multiple conditions can sometimes mimic androgenetic alopecia, a punch biopsy was discussed as a possible future diagnostic test if needed.
Diagnosis: Androgenetic Alopecia (Female Pattern Hair Loss)
Androgenetic alopecia is the most common cause of chronic hair thinning in women.
It occurs due to genetic and hormonal influences that gradually miniaturize hair follicles.
Common signs include:
Diffuse thinning over the crown
Widened midline part
Progressive decrease in hair density
Family or hormonal influence
Without treatment, the condition is slowly progressive over time.
Why Hair Loss Can Return After Stopping Treatment
Hair loss medications help maintain and protect existing hair follicles, but they do not permanently cure the underlying genetic condition.
When therapy is stopped:
Hormonal effects on hair follicles return
Follicles may begin shrinking again
Hair thinning can gradually worsen
This explains why many patients experience recurrent thinning after stopping medications like spironolactone or minoxidil.

Dose: 50 mg twice daily

Medication Counseling
The patient was counseled about possible side effects, including:
Breast tenderness
Menstrual irregularities
Fatigue
Dizziness
Elevated potassium levels
Patients taking spironolactone may require periodic blood tests to monitor kidney function and potassium levels.
Laboratory Testing
To ensure medication safety, a Comprehensive Metabolic Panel (CMP) was ordered to evaluate:
Kidney function
Electrolytes
Potassium levels
Monitoring labs are important when using medications that may affect electrolyte balance.
Future Treatment Considerations
The patient is also considering additional therapies in the future, including:
Platelet-Rich Plasma (PRP) therapy
