Addressing Male Pattern Hair Loss in a 33-Year-Old Patient in Houston, TX

Hair loss is a common concern among men in their 30s, often causing significant distress and impacting self-confidence. At Reena Jogi MD, located in the heart of Houston and serving the greater Katy area, we specialize in diagnosing and treating various forms of alopecia, including androgenetic alopecia—commonly referred to as male pattern hair loss. In this case spotlight, we share the journey of a 33-year-old male patient navigating progressive hair thinning and exploring treatment options for long-term scalp health.

Initial Concerns and History

The patient, a healthy 33-year-old male, presented to our dermatology practice with a chief complaint of gradual hair loss over the past year. He described his condition as moderate and diffuse, noting a progressive thinning pattern, especially on the scalp. Prior to visiting our clinic, he had trialed over-the-counter topical minoxidil, along with natural peppermint and rosemary oils, for about one month without noticeable improvement.

He reported no significant medical issues and denied any associated symptoms like itching, redness, or scalp discomfort. The use of a dermatoscope during the clinical exam confirmed visible thinning most consistent with androgenetic alopecia (AGA).

Clinical Findings and Diagnosis

Upon evaluation, the hair pull test was positive—indicating active shedding. The scalp showed signs of diffuse, non-scarring hair loss without inflammation or scaling, supporting a diagnosis of androgenetic alopecia. We discussed the hereditary and hormonal basis of AGA, explaining how it typically follows a genetically pre-determined pattern that gradually progresses over time.

Treatment Options Reviewed

At Reena Jogi MD, we prioritize personalized care by offering both medical and cosmetic hair restoration solutions. For this patient, we reviewed the following evidence-based treatment modalities:

Given the patient's desire for a non-invasive and effective regimen, he opted to start oral minoxidil 2.5 mg daily in conjunction with topical minoxidil 5% solution. He declined finasteride at this time.

Oral Minoxidil Counseling and Safety

Oral minoxidil is an off-label, increasingly popular treatment for AGA, especially in patients who do not respond well to topical options. We counseled the patient on possible side effects, including:

  • Shortness of breath

  • Swelling in the feet or ankles

  • Low blood pressure or dizziness

  • Unwanted body hair growth

The patient understood these risks and will monitor for any adverse effects. He was advised that initial shedding may occur within the first 8–10 weeks and that results typically become noticeable after 3–6 months of consistent use.

Future Plan and Follow-Up

The patient plans to follow up in six months to assess progress and explore potential cosmetic treatments like PRP or AlmaTED if needed. He was encouraged to maintain a healthy scalp regimen, including gentle cleansing with sulfate-free shampoos and limiting stress, which can exacerbate hair loss.
