AlmaTED Therapy for Male Pattern Hair Loss After Hair Transplant

Patient Presentation

A 60-year-old male established patient presented to Reena Jogi, MD for follow-up evaluation of chronic hair loss on the scalp. His hair loss was described as generalized, moderate in severity, and slowly progressive, having developed over many years. The patient has a history of androgenetic alopecia and is status post hair transplant, but continues to experience diffuse thinning and patterned hair loss.

He expressed interest in non-surgical options to further improve hair density and quality, specifically asking about AlmaTED therapy as an adjunct to his existing treatment regimen.

Clinical Examination

A comprehensive dermatologic examination was performed, including detailed inspection of the scalp and hair, as well as examination of the head, face, conjunctivae, eyelids, lips, nose, and neck.

The patient appeared well developed, well nourished, and in no acute distress. Scalp examination revealed diffuse non-scarring hair loss with a patterned distribution, most consistent with male androgenetic alopecia, commonly affecting the vertex and frontotemporal regions.

Diagnosis

Based on clinical findings, the patient was diagnosed with:

  • Androgenetic Alopecia (Male Pattern Hair Loss)

  • Diffuse Non-Scarring Hair Loss of the Scalp

This presentation is common in men who have previously undergone hair transplantation but continue to experience progressive miniaturization of native hair follicles.

Treatment Discussion and Plan

The patient is currently taking finasteride 1 mg daily and tolerating it well. During the visit, treatment options were reviewed in detail, including topical minoxidil, platelet-rich plasma (PRP), low-level laser therapy, and AlmaTED.

AlmaTED Hair Restoration Therapy

After counseling, the patient elected to proceed with AlmaTED therapy, a non-invasive ultrasound-based hair restoration treatment designed to stimulate hair growth and improve scalp health.

Key counseling points included:

  • AlmaTED requires no downtime or injections

  • Patients should avoid washing their hair for 24 hours after each treatment

  • A recommended protocol of three treatments spaced one month apart

  • Maintenance treatments every six months for optimal long-term results

Medication Review

The option of switching from finasteride to dutasteride 0.5 mg was discussed, including potential side effects such as decreased libido, erectile dysfunction, ejaculatory changes, gynecomastia, and mood changes. After reviewing risks and benefits, the patient elected to continue finasteride and reassess medication adjustments after evaluating his response to AlmaTED therapy.

Counseling and Expectations

The patient was counseled that male pattern hair loss is genetically determined and slowly progressive, even after hair transplantation. Combination therapy—addressing both transplanted and native hair—is often necessary to maintain results.

He was advised to contact the office if hair loss worsens, fails to improve, or if any side effects from treatment occur.
