Evaluating a New Facial Growth: Acne Cyst vs Skin Lesion in Katy and Houston, Texas

New or changing facial growths can be concerning, especially when they become irritated or painful. At Reena Jogi, MD, we routinely evaluate facial lesions to determine whether they represent benign skin growths, inflammatory acne cysts, or other conditions that may require biopsy or procedural treatment. This case highlights a comprehensive dermatologic evaluation and targeted treatment approach.

Patient Overview

A 53-year-old female, established with our practice, presented with a new, irritated growth on the right cheek. The lesion was moderate in severity and had recently appeared, prompting evaluation to rule out concerning pathology and determine appropriate treatment.

Comprehensive Skin Examination

A full-body skin examination was performed, including:

  • Scalp and hair

  • Face, eyelids, lips, nose, and ears

  • Neck, chest, abdomen, and back

  • Upper and lower extremities, hands, feet, and nails

  • Genital, groin, and buttock areas

The patient appeared well-developed, well-nourished, alert, and in no acute distress. A dermatoscope was used to enhance diagnostic accuracy.

Benign Nevi Findings

During the exam, the patient was noted to have benign nevi on the inferior thoracic spine. These moles were:

  • Regular

  • Symmetrical

  • Evenly pigmented

Counseling and recommendations included:

The patient was advised to contact the office if any moles change in size, shape, color, or begin to itch, bleed, or burn.

Facial Growth Diagnosis: Inflammatory Acne Cyst

The primary concern on the right inferior lateral buccal cheek was diagnosed as an inflammatory acne cyst. While benign in nature, these lesions can occasionally mimic other conditions, including dental sinus tracts. We discussed that if the lesion failed to resolve, a biopsy could be considered to rule out alternate causes.

Treatment with Intralesional Kenalog

To reduce inflammation and promote rapid resolution, the patient underwent intralesional Kenalog (ILK) injection:

  • Medication: Kenalog 2.5 mg/cc

  • Total volume injected: 0.2 cc

  • Number of lesions treated: 1

The risks, including localized skin atrophy, were reviewed in detail prior to treatment.

Patient Counseling and Expectations

The patient was counseled that:

  • Isolated inflammatory acne cysts are common and easily treatable

  • Improvement is expected following ILK treatment

  • Topical or systemic antibiotics may be considered if recurrence occurs

She was advised to contact the office if the lesion worsened or failed to resolve.
