Hair thinning accompanied by an itchy, flaky scalp can be frustrating, especially when previous treatments have not worked. A 29-year-old woman recently visited our dermatology clinic with two years of progressive hair thinning and persistent scalp irritation despite using topical steroids.
She had also recently been diagnosed with polycystic ovary syndrome (PCOS), raising questions about whether hormonal changes could be contributing to her hair loss.
Diagnosing Two Scalp Conditions
After a comprehensive scalp examination using dermoscopy, two conditions were identified:
1. Seborrheic Dermatitis: A chronic inflammatory scalp condition causing itching, flaking, and pink-to-orange scaly patches. The patient had previously tried multiple treatments, including topical steroids, without sufficient improvement.
2. Androgenetic Alopecia (Female Pattern Hair Loss): A progressive form of hair thinning associated with genetic and hormonal factors. Her recent PCOS diagnosis was particularly relevant because increased androgen activity can contribute to female pattern hair loss.
Although these conditions can occur together, seborrheic dermatitis and androgenetic alopecia require different treatment approaches.
Treating an Itchy, Flaky Scalp
To manage her persistent seborrheic dermatitis, the patient was prescribed Zoryve (roflumilast) 0.3% foam, a nonsteroidal prescription treatment.
Her treatment plan included:
Washing with Vichy Dercos Anti-Dandruff Shampoo as directed.
Applying Zoryve foam once daily to affected scalp areas after washing and drying.
Following up if symptoms failed to improve.
Seborrheic dermatitis is often chronic, but appropriate treatment can help control itching, scaling, and flare-ups.
What Are the Treatment Options for PCOS-Related Hair Loss?
For her androgenetic alopecia, several options were discussed, including topical or oral minoxidil, spironolactone, finasteride, dutasteride, PRP hair restoration, Alma TED, and low-level laser therapy.
Because treatment depends on medical history, hormonal factors, and individual goals, the patient chose to consider her options before beginning a specific hair restoration treatment.
A small, benign scalp mole was also identified during the examination. Observation and routine skin checks were recommended.
