Hair thinning combined with recurring scalp bumps can be frustrating, especially when previous treatments have provided only temporary relief. A 40-year-old man recently visited our dermatology clinic with years of progressive hair loss and recurring scalp bumps.
With a family history of hair loss affecting his father, he wanted to explore treatment options for both his thinning hair and persistent scalp irritation.
Diagnosing Male Pattern Hair Loss and Scalp Folliculitis
After a comprehensive scalp examination, two conditions were identified:
1. Androgenetic Alopecia (Male Pattern Hair Loss): A genetically influenced condition causing gradual, progressive thinning. The patient had previously tried over-the-counter minoxidil and was currently using a Keeps 5-in-1 supplement.
2. Scalp Folliculitis: An inflammatory condition affecting hair follicles, causing small bumps and pustules. He had previously experienced improvement with isotretinoin (Accutane), but the bumps returned after stopping treatment.
Although these conditions can occur together, male pattern hair loss is primarily driven by genetic and hormonal factors, while folliculitis involves inflammation of the hair follicles.
Treating Recurring Scalp Bumps
To manage his folliculitis, a personalized topical treatment regimen was prescribed:
Plexion cleanser: Wash affected scalp areas once daily.
Clindamycin 1% gel: Apply to affected areas in the morning.
Tretinoin 0.025% gel: Apply a pea-sized amount to affected scalp areas at bedtime.
If his folliculitis does not improve adequately, low-dose isotretinoin may be considered in the future.
What Are the Best Options for Advanced Male Pattern Hair Loss?
For his androgenetic alopecia, several treatments were discussed, including oral minoxidil, finasteride, dutasteride, nutritional supplements, low-level laser therapy, PRP injections, and hair transplantation.
Because of the extent and duration of his hair thinning, hair transplantation was discussed as the option most likely to provide a substantial cosmetic improvement in his individual case.
The patient elected to continue his current hair supplement for now. Baseline photographs were taken to monitor progression, with follow-up recommended in approximately six months.
