Case Report: Successful Acne Control With Low-Dose Isotretinoin in an Adult Male

Introduction

Acne is often thought of as a teenage condition, but many adults continue to struggle with persistent or recurrent breakouts well into their 30s and beyond. At Reena Jogi, MD, we frequently see adult patients whose acne has not responded adequately to topical treatments or oral antibiotics. This case highlights the successful management of adult acne using low-dose isotretinoin, with close monitoring and excellent tolerance.

Patient Background

A 37-year-old male presented for follow-up care for acne vulgaris at our dermatology clinic serving Katy and Houston, Texas. He was initially evaluated on November 11, 2025, at which time oral isotretinoin 10 mg three times daily was initiated due to persistent inflammatory acne.

The patient returned for routine follow-up to assess treatment response, side effects, and medication tolerance.

Clinical Findings

A focused dermatologic examination of the face was performed. The patient appeared well-developed and well-nourished, was alert and oriented, and in no acute distress. A dermatoscope was utilized to assist with lesion evaluation.

At this visit:

  • No active inflammatory breakouts were observed

  • Significant improvement was noted compared to the prior visit

  • No new nodules, cysts, or scarring were identified

Treatment Response

The patient reported excellent tolerance of isotretinoin therapy. Aside from very mild dryness, he denied any significant side effects, including:

  • Mood changes

  • Headaches or vision issues

  • Joint or muscle pain

  • Gastrointestinal symptoms

Importantly, the patient experienced no acne flares during the past month, indicating an early positive response to therapy.

Given the favorable response, the treatment plan was to continue isotretinoin 10 mg three times daily.

Ongoing Monitoring & Safety

Because isotretinoin is a high-risk medication, appropriate monitoring is essential. As part of standard isotretinoin management at our Katy and Houston dermatology offices, the following were addressed:

  • Laboratory monitoring:

    • Hepatic Function Panel

    • Triglycerides

  • iPLEDGE compliance:

    • Patient confirmed and prescription authorized

  • Patient counseling included:

    • Avoiding blood donation

    • Not sharing medication

    • Awareness of potential side effects such as dryness, hair shedding, joint aches, mood changes, and lab abnormalities

    • Contacting the office immediately for severe headaches, vision changes, depressive symptoms, or abdominal pain

The patient was provided a lab slip and instructed to return in one month for continued evaluation.

Acne Education & Skin Care Counseling

Comprehensive acne counseling was reviewed, including realistic expectations and supportive skin care. The patient was advised that improvement typically continues over 2–3 months, with up to 60–80% clearance.

Recommended non-comedogenic products included:

  • Cleansers: Cetaphil Oil Control, CeraVe Foaming Cleanser, Neutrogena Oil-Free Acne Wash, SkinMedica AHA/BHA Cleanser

  • Moisturizers & Sunscreen: CeraVe PM, Cetaphil Oil Control, EltaMD UV Clear

Benzoyl peroxide-based moisturizers were also discussed as adjunctive therapy if needed.
