Patient Overview
A 53-year-old female visited our clinic with two main concerns:
Diffuse hair thinning over the past 5–6 years

She had been taking Nutrafol supplements for hair health and using hydroquinone for pigmentation. She reported a family history of fine, thinning hair on both maternal and paternal sides.
Hair Loss Diagnosis and PRP Treatment
A scalp examination and positive hair pull test confirmed a diagnosis of androgenetic alopecia (AGA) with no scarring. We discussed multiple treatment options, including:
Oral or topical minoxidil
Finasteride or spironolactone (for appropriate candidates)
Low-level laser therapy

PRP Procedure
Blood was drawn and processed in a centrifuge to isolate platelet-rich plasma.
7 cc of PRP was injected into the scalp using a subdermal technique.
Pain control was provided with Zimmer cooling, and the procedure was well-tolerated.
Post-procedure instructions included using sulfate-free shampoos, continuing any hair loss supplements, and avoiding hair dye for 2–3 days.
The patient also used Pro-Nox™ (a patient-controlled inhaled analgesia) for comfort during the procedure.
Melasma Evaluation and Treatment
The patient had ill-defined hyperpigmented patches in the cheek area, consistent with melasma. We discussed:
Triggers: Sun exposure, hormonal changes, pregnancy, and certain medications
Treatment options:
Topical bleaching agents (hydroquinone)
Retinoids
Vitamin C
Superficial chemical peels
Non-ablative Fraxel laser
Strict sun protection with SPF reapplication every 2 hours
She was prescribed a compounded topical hydroquinone cream from a specialty pharmacy to be used for 2–3 months before reassessment.
