Female Androgenetic Alopecia with Telogen Effluvium: PRP Hair Restoration in Katy and Houston, Texas

Patient Presentation
A 26-year-old female, established patient, presented for evaluation of chronic scalp hair loss. The hair loss had been moderate in severity, gradual in onset, and present for approximately 10 years. The patient denied associated scalp symptoms such as itching, pain, or scaling.
Her current treatment regimen included minoxidil and spironolactone 25 mg daily. She reported receiving PRP treatments at an outside dermatology office in New York and wished to continue maintenance PRP therapy after recently relocating.
Physical Examination
A comprehensive dermatologic examination was performed, including evaluation of the scalp with hair inspection, head and face, conjunctivae and lids, lips, nose, and neck. The patient appeared well developed and well nourished, was alert and oriented, and in no acute distress.
Scalp examination demonstrated diffuse, non-scarring hair thinning with a patterned distribution, consistent with female pattern hair loss.
Diagnosis
Androgenetic Alopecia with Associated Telogen Effluvium (L64.8)
Based on clinical history and examination findings, the patient was diagnosed with androgenetic alopecia, with an associated component of telogen effluvium, contributing to diffuse shedding.
Treatment Discussion
A detailed discussion was held regarding treatment options for female pattern hair loss, including:

Treatment Plan
Medication Adjustments
The patient elected to optimize her current medical regimen:
Oral minoxidil increased to one full tablet daily
Spironolactone increased from 25 mg to 50 mg once daily
Comprehensive counseling was provided regarding potential side effects of oral minoxidil, including unwanted hair growth, dizziness, edema, and cardiovascular symptoms, as well as spironolactone risks such as electrolyte imbalance, teratogenicity, and the need for periodic laboratory monitoring.
Platelet-Rich Plasma (PRP) Treatment
The patient underwent PRP injection therapy for hair loss during this visit.

The patient’s blood was drawn under sterile conditions, centrifuged to isolate platelet-rich plasma, and injected into the scalp at the subdermal level. Pain control was achieved with Zimmer. The procedure was well tolerated without complications.
Post-procedure care instructions were reviewed, including hair washing guidance, avoidance of hair dye for several days, and continuation of hair loss medications.
Follow-Up
The patient will return in 4 months for her next PRP maintenance treatment and reassessment of hair density and shedding.
