Managing Chronic Androgenetic Alopecia in a Young Male Patient in Katy & Houston, Texas
At Reena Jogi, MD, our dermatology team frequently treats men struggling with gradual hair thinning. This case features a 26-year-old male patient from the Katy and Houston, Texas area who presented with generalized hair loss consistent with androgenetic alopecia (male pattern baldness).
Patient Background
The patient reported gradual, moderate hair loss over the past four years, with a family history of hair loss on his paternal side. His past treatments included:
Topical Rogaine® (minoxidil) used intermittently
Oral finasteride for six months, followed by topical finasteride
Low-level laser therapy for 4–6 months
Rosemary oil at night
While his hair loss remained stable during oral finasteride treatment, he did not notice regrowth.
Clinical Findings
On examination, the patient displayed:
Patterned hair thinning on the posterior mid-parietal scalp
Diffuse non-scarring hair loss
No signs of scalp inflammation or scarring
Diagnosis
Androgenetic Alopecia – Chronic, genetically determined male pattern hair loss.
Treatment Plan
Oral Medications:
After discussing all treatment options, including PRP, oral dutasteride, and hair transplantation, the patient elected to begin:
Oral Minoxidil 2.5 mg daily – To stimulate hair regrowth and slow shedding
Oral Finasteride 1 mg daily – To reduce dihydrotestosterone (DHT) levels and prevent further hair loss
The patient was counseled on potential side effects of both medications, the typical 6-month timeline to notice improvement, and the possibility of increased shedding in the first 8–10 weeks with minoxidil.
Supportive Therapies:
Continue OTC rosemary oil at night if desired
Consider PRP treatments in the future for additional follicle stimulation
Maintain a healthy scalp with gentle cleansing and minimal heat styling
Follow-Up Plan
The patient will return in 6–9 months for reassessment, with baseline photographs taken at this visit to monitor progress.
