Teen Acne Management in Katy and Houston, Texas

Introduction
At Reena Jogi MD, we understand that acne in teens can be both physically uncomfortable and emotionally challenging. Our goal is to create personalized treatment plans that are safe, effective, and tailored to each patient’s needs. This case report highlights the follow-up care of a 14-year-old female patient from the Katy and Houston, Texas area who is managing her acne with topical therapies.

Case Overview
The patient initially visited our office on July 11, 2024, for evaluation of facial acne. At that time, she began treatment with:

  • Tretinoin 0.025% topical cream – applied 2-3 nights per week, increasing as tolerated.

  • Winlevi® (clascoterone) 1% topical cream – applied each morning.

She returned for follow-up to review her progress and discuss further options.

Examination & Findings
A thorough examination of the scalp, face, ears, lips, and upper extremities was performed. The patient appeared well-developed, well-nourished, alert, and in no distress. Facial acne was present but stable on her current regimen.

Treatment Discussion
We reviewed several potential adjustments to her acne management plan, including:

The patient previously discontinued isotretinoin due to elevated triglycerides and illness (flu and pneumonia) during treatment. Given her stability on topicals and preference to avoid systemic medications, we decided to:

  • Continue Winlevi® 1% topical cream (QAM)

  • Increase tretinoin to 0.05% topical cream (QHS)

  • Maintain supportive skincare routines

Acne Care Education
We reinforced important acne care tips:

  • Use non-comedogenic cleansers, moisturizers, and cosmetics

  • Apply broad-spectrum SPF 30+ sunscreen daily

  • Be patient — noticeable improvement often takes 2-3 months

  • Contact our office if acne worsens, scars develop, or cystic lesions increase

Counseling on Topical Medications

  • Tretinoin: Apply a pea-sized amount at bedtime, 30 minutes after cleansing, on completely dry skin to reduce irritation.

  • Winlevi®: Be aware of possible mild redness, scaling, itching, or stinging.

Outcome & Plan
The patient’s acne remains well-controlled with topical therapy. We will monitor progress and adjust treatment as needed. She will return as necessary for follow-up.
