Platelet-Rich Plasma (PRP) Maintenance for Longstanding Female Hair Loss: Case Report from Katy & Houston, Texas

Case Summary: 77-Year-Old Female with Chronic Hair Loss
Our patient, a healthy and active 77-year-old woman, presented with diffuse, moderate hair thinning across the scalp. The hair loss had been gradually progressing over many years. She previously tried oral minoxidil and finasteride under the care of another dermatologist, and had undergone three PRP sessions elsewhere with noticeable improvement.
During her visit, she expressed interest in continuing PRP therapy but declined starting or restarting any oral hair loss medications.
Diagnosis: Androgenetic Alopecia with Underlying Telogen Effluvium
A dermatoscopic scalp exam revealed signs consistent with androgenetic alopecia (female pattern hair loss) and chronic telogen effluvium—a condition where a higher-than-normal percentage of hairs enter the shedding phase. The hair pull test was positive, confirming ongoing shedding.
She also had mild seborrheic dermatitis on the scalp and in the ears, which can contribute to inflammation and hair fragility if left untreated.
Treatment Plan
After reviewing all options—including topical minoxidil, supplements, low-level laser therapy, and hair transplantation—the patient chose to proceed with PRP maintenance.
PRP Procedure
Blood was drawn, processed in a centrifuge to extract platelet-rich plasma, and carefully injected into targeted scalp areas.
A total of 7 cc of PRP was delivered into the subdermal layer using sterile technique and local cooling for comfort.
The patient tolerated the procedure well without complications.
She was advised to:
Wash hair in a warm shower after the procedure
Use sulfate-free shampoos
Avoid coloring the hair for 2–3 days
Continue any topical treatments or supplements she chooses
Seborrheic Dermatitis Care
Continue fluocinolone 0.01% oil for inside the ears as needed
Use clobetasol 0.05% foam for scalp flares, limited to short-term use
