Early-Onset Male Hair Loss Treated Successfully with Oral Medications and Scalp Care: A Case Report from Reena Jogi MD

Case Summary: 18-Year-Old Male with Generalized Hair Loss
A healthy 18-year-old male presented to our clinic with a two-year history of gradual, generalized scalp hair thinning. The patient had a family history of hair loss (father and sister affected), and had been using spironolactone 25 mg for a month prior to his visit. However, because spironolactone carries a significant risk of gynecomastia (breast enlargement) in men, we recommended stopping it immediately.
Clinical Evaluation and Diagnosis
On examination, the scalp displayed diffuse non-scarring hair loss, consistent with androgenetic alopecia. A dermatoscope was used to confirm diagnosis. A hair pull test was positive, supporting active hair shedding.
In addition, the patient exhibited mild seborrheic dermatitis—characterized by pink-orange scaly plaques on the scalp. This common, chronic scalp condition often exacerbates hair loss and was addressed concurrently.
Treatment Plan
After a detailed discussion of treatment options, the patient elected to begin a two-pronged oral therapy regimen:
Minoxidil 2.5 mg oral tablets, initially at half a tablet daily, titrating up to one tablet daily as tolerated. He was educated on potential temporary shedding in the first 8–10 weeks and possible side effects (e.g., swelling, dizziness, unwanted hair growth).
Finasteride 1 mg oral tablets daily, with counseling about potential side effects such as low libido or mood changes.
For seborrheic dermatitis, we initiated a topical regimen:
Ketoconazole 2% shampoo (used 1–3 times weekly)
Fluocinonide 0.05% topical solution, applied during flare-ups only.
Educational handouts were provided, baseline photos were taken, and the patient will follow up in 6 months to assess progress and adjust treatments as needed.

Starting evidence-based therapy like oral minoxidil and finasteride at the first signs of thinning can prevent extensive future loss.
