PRP and Oral Minoxidil for Longstanding Hair Loss in a 35-Year-Old Male: Treatment Options at Reena Jogi MD in Houston and Katy, Texas

At Reena Jogi MD, we frequently see patients who have struggled with hair loss for years and are ready to explore advanced solutions. Recently, a 35-year-old male patient visited our Houston dermatology office for help with diffuse androgenetic alopecia—a condition he had been managing for over 13 years with minimal success.

Despite trying topical finasteride and minoxidil in the past, inconsistent use limited his results. He also reported a family history of hair loss on his maternal side. Frustrated with the progression, he sought an effective treatment plan tailored to his needs.

Clinical Evaluation and Diagnosis

Upon examination using a dermatoscope, we observed classic signs of androgenetic alopecia (AGA): diffuse hair thinning on the scalp, especially on the vertex and frontotemporal areas. A positive hair pull test indicated active hair shedding.

Given the chronicity and progression of his hair loss, we discussed performing a scalp punch biopsy to confirm the diagnosis and rule out other types of alopecia. However, clinical findings strongly favored AGA without scarring or inflammation.

Personalized Treatment Plan: PRP and Oral Minoxidil

After an in-depth consultation, we created a comprehensive hair restoration plan that included:

  • Platelet-Rich Plasma (PRP) Injection
As a non-surgical, regenerative therapy, PRP helps stimulate hair growth by injecting concentrated growth factors directly into the scalp. The patient received his first PRP session at our office. We used Zimmer cooling technology for pain control, and a total of 7 cc of PRP was injected into targeted areas. He tolerated the procedure well and will return in 1 month for his second session.

  • Oral Minoxidil 2.5 mg Daily
Oral minoxidil offers an alternative to the topical form and is increasingly used to treat male pattern hair loss. The patient was counseled on potential side effects (such as swelling, dizziness, or unwanted hair growth), and he agreed to begin therapy with regular monitoring.

Addressing Seborrheic Dermatitis

During the scalp exam, the patient also showed signs of seborrheic dermatitis, a common inflammatory scalp condition that can worsen hair loss. Treatment included:

  • Ketoconazole 2% Shampoo (prescription)

  • Fluocinonide 0.05% Solution for flare-ups

  • Over-the-counter anti-inflammatory shampoos on alternating days

These steps are designed to reduce inflammation and support a healthy scalp environment for optimal hair regrowth.
