PRP Therapy for Male Pattern Hair Loss

Patient Presentation

A 52-year-old male established patient presented to Reena Jogi, MD for evaluation and treatment of chronic hair loss on the scalp. He reported generalized hair thinning that was moderate in severity, gradual in onset, and present for many years. The patient denied scalp itching, pain, or burning.

He had not previously tried any prescription or procedural treatments and noted a strong family history of hair loss, including both his brother and mother. He was interested in initiating treatment and specifically requested platelet-rich plasma (PRP) therapy.

Clinical Examination

A focused dermatologic examination was performed, including detailed inspection of the scalp and hair, as well as examination of the face and eyelids. The patient appeared well developed, well nourished, and in no acute distress.

Scalp examination demonstrated diffuse non-scarring hair loss with a patterned distribution, consistent with androgenetic alopecia (male pattern hair loss), primarily affecting the vertex and frontal scalp.

Diagnosis

Based on the patient’s clinical presentation, family history, and examination findings, he was diagnosed with:

  • Diffuse Non-Scarring Hair Loss of the Scalp

Treatment Discussion and Counseling

The patient was counseled extensively on androgenetic alopecia, including its genetic nature and slowly progressive course. Treatment options were reviewed in detail, including:

The patient was informed that PRP is a cosmetic procedure not covered by insurance and requires out-of-pocket payment. Risks, benefits, and realistic expectations were reviewed, and the patient expressed understanding.

The patient elected to proceed with PRP therapy during the visit.

Written consent was obtained, and risks—including pain, infection, scarring, and incomplete improvement—were reviewed. Blood was drawn using sterile technique, centrifuged to isolate platelet-rich plasma, and injected into the scalp at the subdermal level. Pain control was achieved using Zimmer cooling, and the patient tolerated the procedure well without complications.

Post-Procedure Instructions

The patient was advised to:

  • Wash hair in a warm shower

  • Use sulfate-free shampoos

  • Continue any hair loss prescriptions or supplements

  • Continue topical minoxidil if using

  • Avoid hair coloring for 2–3 days after treatment

A follow-up visit was scheduled in 1 year, with the understanding that additional PRP sessions may be recommended based on response.
