Case Report: 57-Year-Old Female with Long-Standing Hair Loss

A 57-year-old female presented for evaluation of generalized scalp hair thinning that had been gradually progressing for several years.

Patient History

The patient reported moderate hair loss affecting the scalp and had already tried many treatments, including:

Supplements

  • Biotin

  • Force Factor hair supplements

  • Hair skin and nail supplements

Hair Growth Supplements

  • Nutrafol

  • Viviscal

Devices

  • iRestore laser helmet

Hair Products

  • Spoiled Child hair products

  • Moerie shampoo and conditioner

  • Keranique

  • Rice water treatments

Despite trying numerous therapies, she continued to experience persistent diffuse hair thinning.

The patient also noted that hair shedding worsened after briefly starting a GLP-1 medication, which she has since discontinued.

Dermatologic Examination

A detailed scalp examination was performed.

Clinical Findings

The patient demonstrated:

This condition is the most common cause of hair loss in women, particularly after menopause.

Diagnosis: Androgenetic Alopecia (Female Pattern Hair Loss)

Androgenetic alopecia occurs when hair follicles gradually shrink due to hormonal and genetic factors.

Common signs include:

  • Diffuse thinning across the scalp

  • Widening of the central part

  • Preservation of the frontal hairline

  • Gradual progression over time

Because this condition is genetically driven, many over-the-counter products alone may not be sufficient.

Why Many Hair Products Do Not Work

Patients frequently try multiple shampoos, supplements, and devices before seeking medical care. While some products can support hair health, they do not always address the underlying cause of follicle miniaturization.

For patients with androgenetic alopecia, treatment often requires medical therapy combined with procedural treatments.

Treatment Options Discussed

Oral Minoxidil

  • Dose: 1/2 tablet daily (2.5 mg tablet)

Possible side effects discussed included:

  • Dizziness

  • Palpitations

  • Leg swelling

  • Fluid retention

  • Unwanted hair growth

Platelet-Rich Plasma (PRP) Treatment

The patient also chose to begin PRP hair restoration therapy.

Procedure Details

  • Blood was drawn and processed to isolate platelet-rich plasma

  • PRP was injected into the scalp

  • 7 cc of PRP was administered

  • Zimmer cooling device was used for comfort

PRP contains growth factors that may help stimulate dormant hair follicles and improve hair density.

PRP Treatment Plan

PRP therapy is typically performed in a series of treatments.

Recommended schedule:

  • 1 treatment per month

  • Series of 3–4 treatments initially

  • Maintenance sessions every 6–12 months

The patient will return in 1 month for PRP treatment #2.

Post-Procedure Care

Following PRP treatment, patients are advised to:

  • Wash hair with sulfate-free shampoo

  • Avoid hair coloring for 2–3 days

  • Continue prescribed hair loss medications

  • Resume normal daily activities

Minor side effects may include:

  • Temporary scalp tenderness

  • Mild swelling

  • Pinpoint bleeding at injection sites

Serious complications are extremely rare.
