“Why Is My Hair Thinning Even Though I Tried Topical Finasteride and Minoxidil?”
Hair thinning is one of the most common concerns patients bring to our dermatology clinic. Many patients attempt over-the-counter or online treatments first but may not see consistent improvement. At Reena Jogi, MD Dermatology in Katy, Texas and Houston, Texas, we help patients understand the underlying cause of their hair loss and develop a personalized treatment plan.
A recent patient asked during his visit:
“Why is my hair still thinning even though I tried topical finasteride and minoxidil?”
This case illustrates how androgenetic alopecia (male pattern hair loss) can progress when treatments are used inconsistently and how targeted therapy can help stabilize hair thinning.
Case Report: 39-Year-Old Male with Hair Thinning
A 39-year-old male presented as a new patient for evaluation of progressive scalp hair loss.
Patient History
The patient reported:
Gradual hair thinning over approximately 1 year
Moderate generalized hair loss
Family history of hair loss in his maternal grandfather
Previous treatment with topical finasteride and minoxidil
However, the patient acknowledged that he had not been consistent with treatment, which can significantly affect results.
Dermatologic Examination
A comprehensive scalp examination was performed, including dermatoscopic evaluation of the hair follicles.
Clinical Findings
The examination revealed:
Diffuse thinning of scalp hair
Positive hair pull test
Pattern consistent with androgenetic alopecia
Because hair loss conditions can occasionally overlap, a punch biopsy was discussed as a possible future diagnostic tool if needed.
Diagnosis: Androgenetic Alopecia (Male Pattern Hair Loss)
Androgenetic alopecia is the most common cause of hair loss in men and is influenced by genetic and hormonal factors.
Typical features include:
Thinning at the crown (vertex)
Recession at the frontotemporal hairline
Progressive follicle miniaturization
Family history of hair loss
Without treatment, hair thinning typically progresses over time.
Why Hair Loss Treatments Sometimes Fail
Many patients try treatments but stop too early or use them inconsistently. Hair restoration therapies require long-term, consistent use to be effective.
Hair growth cycles are slow, and treatments often require:
3–4 months to slow shedding
6 months to see early improvement
12 months for maximal results
Stopping therapy prematurely often leads to continued thinning.
Treatment Options Reviewed
During the visit, several treatment options were discussed, including:
Medical Treatments
Oral minoxidil
Finasteride
Topical minoxidil
Procedural Treatments
Additional Options
Low-level laser therapy
Hair supplements
Hair transplantation
Because PRP and AlmaTED are cosmetic procedures, they are typically not covered by insurance.
Patient’s Treatment Plan
After reviewing all treatment options, the patient elected to begin a topical compounded therapy.
Prescription Topical Treatment
A customized formulation was prescribed containing:
Minoxidil 7%
Tretinoin 0.0125%
Finasteride 0.1%
This compounded formula helps:
Stimulate hair growth
Improve medication penetration
Reduce hormonal follicle miniaturization
The medication is applied once daily to the scalp.
Future Treatment Considerations
The patient is also considering PRP therapy in the future.
PRP involves:
Drawing a small amount of the patient’s blood
Processing it to concentrate growth factors
Injecting platelet-rich plasma into the scalp
PRP can help stimulate dormant follicles and improve hair density when combined with medical therapy.
Follow-Up Plan
Hair restoration treatments require consistent use and monitoring.
The patient will return for follow-up in:
6 months to evaluate treatment progress.
During follow-up visits, we assess:
Hair density
Hair shedding
Treatment tolerance
Additional treatment options if needed
