Case Report: 39-Year-Old Male with Hair Thinning

A 39-year-old male presented as a new patient for evaluation of progressive scalp hair loss.

Patient History

The patient reported:

  • Gradual hair thinning over approximately 1 year

  • Moderate generalized hair loss

  • Family history of hair loss in his maternal grandfather

  • Previous treatment with topical finasteride and minoxidil

However, the patient acknowledged that he had not been consistent with treatment, which can significantly affect results.

Dermatologic Examination

A comprehensive scalp examination was performed, including dermatoscopic evaluation of the hair follicles.

Clinical Findings

The examination revealed:

Because hair loss conditions can occasionally overlap, a punch biopsy was discussed as a possible future diagnostic tool if needed.

Diagnosis: Androgenetic Alopecia (Male Pattern Hair Loss)

Androgenetic alopecia is the most common cause of hair loss in men and is influenced by genetic and hormonal factors.

Typical features include:

  • Thinning at the crown (vertex)

  • Recession at the frontotemporal hairline

  • Progressive follicle miniaturization

  • Family history of hair loss

Without treatment, hair thinning typically progresses over time.

Why Hair Loss Treatments Sometimes Fail

Many patients try treatments but stop too early or use them inconsistently. Hair restoration therapies require long-term, consistent use to be effective.

Hair growth cycles are slow, and treatments often require:

Prescription Topical Treatment

A customized formulation was prescribed containing:

  • Minoxidil 7%

  • Tretinoin 0.0125%

  • Finasteride 0.1%

This compounded formula helps:

  • Stimulate hair growth

  • Improve medication penetration

  • Reduce hormonal follicle miniaturization

The medication is applied once daily to the scalp.

Future Treatment Considerations

The patient is also considering PRP therapy in the future.

PRP involves:

  • Drawing a small amount of the patient’s blood

  • Processing it to concentrate growth factors

  • Injecting platelet-rich plasma into the scalp

PRP can help stimulate dormant follicles and improve hair density when combined with medical therapy.

Follow-Up Plan

Hair restoration treatments require consistent use and monitoring.

The patient will return for follow-up in:

6 months to evaluate treatment progress.

During follow-up visits, we assess:
