Why Is My Hair Thinning at the Crown Even Though I’m Using Minoxidil?

This case highlights an important distinction between androgenetic alopecia and Central Centrifugal Cicatricial Alopecia (CCCA)—two conditions that can look similar but require different management approaches.

Case Report: 43-Year-Old Female with Chronic Crown Hair Loss

A 43-year-old female presented as a new patient with generalized scalp hair thinning, most prominent at the crown. Her symptoms had been gradually progressing over several years.

Key History:

  • Gradual onset hair thinning

  • Moderate severity

  • Persistent scalp itching

  • Currently using:

    • HERS topical minoxidil (started in January)

    • Ketoconazole shampoo

She was concerned because despite using minoxidil, the thinning at the crown continued.

Dermatologic Evaluation in Katy & Houston, TX

A comprehensive scalp and skin examination was performed. While evaluation was slightly limited due to a braided hairstyle, findings suggested:

Central Centrifugal Cicatricial Alopecia (CCCA)

More likely than androgenetic alopecia (female pattern hair loss)

The distribution of thinning at the central scalp, along with symptoms of itching and chronic progression, increased suspicion for CCCA, a form of inflammatory scarring hair loss.

What Is Central Centrifugal Cicatricial Alopecia?

CCCA is a chronic inflammatory condition that:

  • Typically begins at the crown (vertex)

  • Gradually expands outward

  • Can cause permanent scarring if untreated

  • Is more commonly seen in women of African descent

  • May be associated with heat styling, tight hairstyles, or chemical relaxers

Unlike androgenetic alopecia, CCCA involves inflammation that destroys hair follicles. This is why early diagnosis and treatment are critical.

Because CCCA is inflammatory and potentially scarring, treatment focuses on reducing inflammation and preventing progression.

The patient’s treatment plan included:

✔ Continue topical minoxidil
✔ Start topical clobetasol solution (anti-inflammatory steroid)
✔ Begin low-dose doxycycline (20 mg twice daily for anti-inflammatory effect)
✔ Topical metformin 10% lotion
✔ Continue ketoconazole shampoo for seborrheic dermatitis
✔ Follow-up in 2–3 months
✔ Consider scalp biopsy if response is inadequate

Low-dose doxycycline was discussed as a long-term anti-inflammatory option that is generally well tolerated and does not significantly disrupt normal flora at sub-antimicrobial dosing.

Concurrent Diagnosis: Seborrheic Dermatitis

The patient also had seborrheic dermatitis of the scalp, contributing to itching and inflammation.

Treatment included:

  • Ketoconazole 2% shampoo (leave on 5–10 minutes before rinsing)

  • Clobetasol scalp solution for flares

Managing scalp inflammation is essential when treating scarring alopecia.

Why Minoxidil Alone May Not Be Enough

Many patients assume that all hair thinning is genetic. However:

  • Androgenetic alopecia = non-scarring, hormonal pattern thinning

  • CCCA = inflammatory, scarring condition

If inflammation is present, minoxidil alone will not stop disease progression. Anti-inflammatory therapy is essential.
